However, I don't see this as a reason to relax. I have analyzed available incident data and noticed something more troubling: the market is already flooded with conditions where such a failure is nearly inevitable. Organizations are massively connecting AI tools without proper inventory, data segmentation, or a robust permissions model.
To me, the typical anti-pattern looks like this: a team takes an internal Slack or Teams bot, connects it to a data warehouse, BI, and BigQuery, and calls it "fast AI automation." In practice, the bot gets an overly broad service account, can read more than necessary, and the prompt logic does nothing to restrict the classes of data it can return to the user.
I have often seen that the problem hides not in the model itself, but in its integration wrapper. The LLM doesn't cause the leak; poor AI architecture does—shared tokens, lack of row-level security, unseparated dev/prod environments, logging of sensitive responses, and a complete absence of policy enforcement between the human prompt and the SQL execution.

My forecast is simple: the survivors will not be the most aggressive teams, but the most architecturally disciplined ones. They will build agents not around full data access, but around predefined scenarios, approved tools, and verifiable actions. It's less spectacular, but it scales.
